Holmes Lake Rehabilitation & Care Center has been a key part of the Lincoln community since its establishment in 1974. Located on a 7-acre property, the facility features 97 skilled nursing beds spread across 51 rooms. Over the decades, it has provided care to countless residents in the area. However, like many seasoned facilities, it has faced its share of challenges.
When ESI began the marketing of Holmes Lake, the occupancy rate was at a low 61%, with fluctuations between 70-85% in years prior. The facility had also been dealing with the difficulties of a 1-star CMS rating—an indicator of ongoing operational hurdles and the increasingly stringent regulatory environment. Despite this, Holmes Lake remained a facility with untapped potential—waiting for the right leadership to bring the community back to life.
