While the portfolio maintained a healthy 83% occupancy, ESI’s deep-dive analysis identified that nearby competitors were capturing a disproportionate share of the local Medicare volume. By providing a clear strategic blueprint for a new operator to strengthen hospital referral relationships and drive the Medicare census from 40 to 58 patients per day, ESI demonstrated significant untapped revenue potential. Furthermore, ESI’s clinical experts identified a major opportunity for clinical and coding realignment. The analysis showed that a new operator could raise Medicare Part A rates by $20–$45 per patient day through more accurate MDS coding and optimized care pathway alignment. This comprehensive positioning allowed the seller to be paid for the future potential of the assets today, resulting in a premium final sale price.
