Lease for Southern Indiana Skilled Nursing Facility

ESI arranged a new lease for a 96-licensed-bed skilled nursing facility in New Harmony, Indiana.

ESI represented the owner in transitioning the property from a long-term tenant that exited Indiana, while successfully bringing a new healthcare operator into the state to ensure the community's continued access to quality skilled nursing care. The new lease commenced in July 2026.


How ESI Added Value to the Client:

ESI’s marketing highlighted New Harmony’s substantial upside for an incoming operator. The facility’s occupancy stood at 43%, well below the 64% average across comparable skilled nursing facilities within a 15-mile radius, presenting a clear census recovery opportunity. ESI identified a path to improve the facility’s Upper Payment Limit split from 70/30 to 80/20, a projected increase of over $360,000 in annual supplemental income. Additional savings of over $100,000 annually were identified in rehab, pharmacy, and administrative staffing costs through renegotiated vendor contracts and operational discipline.
